A Brazilian butt lift (BBL) reshapes and enhances the buttocks using your own fat — removed by liposuction from areas like the abdomen, flanks or back, then carefully transferred to add volume and improve shape. There are no implants, and the liposuction slims the donor areas at the same time. The surgery we arrange in Istanbul leads with one thing above all: safety, because the BBL is the procedure where it matters most.
An honest word on BBL safety — first
The BBL has historically carried the highest mortality rate of any cosmetic procedure, caused by fat being injected too deeply and entering a blood vessel (a fat embolism). That risk is not a reason to avoid the procedure — it is a reason to be uncompromising about how and by whom it is done.
Safe modern BBL practice means:
- injecting fat only above the muscle (subcutaneous), never into it;
- using ultrasound guidance to keep the cannula in the safe plane;
- a board-certified plastic surgeon in an accredited hospital;
- sensible volumes, not extreme ones.
We only work with partner surgeons who follow this safe technique. The cheap, high-volume “BBL package” offered by price-led clinics is precisely the thing an honest agency exists to steer you away from — the saving is taken out of safety.
What a BBL can and can’t do
A BBL adds volume and shape using fat, and improves the waist-to-hip balance because the donor areas are slimmed. It does not lift significantly sagging skin (that is a surgical buttock lift), and it needs you to have enough fat to harvest — very slim patients may not be candidates. A responsible surgeon will tell you honestly if you are not suited.
The procedure, step by step
- Consultation and planning. Your shape, donor fat and goals are assessed and a plan agreed.
- Anaesthesia. General anaesthesia in an accredited hospital with full anaesthetic cover.
- Liposuction. Fat is removed from the planned donor areas and purified.
- Transfer. The fat is injected above the muscle under ultrasound guidance, shaping the buttocks. The operation takes 2–4 hours.
Fat survival — the realistic expectation
Not all transferred fat survives. Typically 60–70% takes long-term; the rest is reabsorbed over the first few months, which is why the result looks fuller at first and settles by around six months. A good surgeon plans for this. The fat that survives is permanent but behaves like fat anywhere — it grows and shrinks with your weight.
Recovery timeline
- Weeks 1–3: No direct pressure on the buttocks — sit on a cushion or your thighs, sleep on your front or side. A compression garment is worn over the liposuction areas.
- Week 2: Most desk work resumes.
- Weeks 4–6: Sitting normalises; light exercise resumes.
- Months 3–6: Swelling settles and the final shape appears.
We plan a 7–10 night stay so the surgeon can review you before you fly.
Risks and side effects
Beyond the fat-embolism risk addressed above — which safe technique minimises — a BBL carries the usual risks of liposuction and fat transfer: bleeding, infection, contour irregularity and partial fat loss. A board-certified surgeon using ultrasound-guided, above-the-muscle technique in an accredited hospital is the single most important safety factor. The ISAPS and BAAPS publish independent BBL safety guidance worth reading first.
Cost of a BBL in Turkey
A BBL in Istanbul starts at an indicative €3,500, including the liposuction that harvests the fat — well below UK or US pricing for comparable surgery and accreditation. Be cautious of unusually cheap offers: with a BBL, the price difference is exactly where safety lives. The figure is a starting range rather than a fixed quote; your firm price follows a surgeon’s review of your case. Compare indicative all-inclusive prices across every procedure in the plastic surgery cost guide.
